Want to Change Your Plan?
If you have a “qualifying event” happen in your life, you’re eligible to revise your insurance plans and flexible spending accounts within 31 days following the event. After 31 days, you must wait for Open Enrollment.
A Qualifying Event includes the birth of a baby, adoption, marriage, divorce, and/or loss of eligibility from another carrier.
Be prepared to provide documentation to support the change in status (e.g. marriage license, birth certificate, divorce decree, etc). If you’re wondering whether your event qualifies, contact Human Resources for more information.
To update your plans, download and complete the form below and submit to Human Resources within 31 days of the qualifying event.

Eligibility & Enrollment
You’re eligible for our benefits package if you are an active full-time employee regularly scheduled to work 30 or more hours per week. You may also enroll your eligible family members for coverage under the same plans you choose for yourself.

Documentation required
To add a spouse:
– Marriage license and
– Filing Page of previous year tax forms or
– Other contractual agreement signed with spouse (please black-out financial information)
To add a child/dependent:
– Copy of certified birth certificate, or adoption paperwork from agency or country
– Legal guardianship: court documents stating the appointed guardianship
