Wellness Program & Screenings
In order to participate in our wellness programs and earn your rebate, you must take a biometric screening each calendar year. You can complete your biometric screening at one of our wellness clinics, or on your own. A completed & signed Wellness Confirmation Form is needed to verify the screening (see below) – the deadline for submitting this each year is December 31st. An employee and/or spouse must complete a preventive care exam with their primary care physician in order to receive the wellness credit, as well as complete the bloodwork for a biometric screening (please see the form below “Schedule Your Own Screening”). If the primary care physician does not consider the bloodwork necessary, this must be indicated on the form. This form, “Wellness Confirmation Form”, must be submitted to Human Resources by 12/31.
